However, many people have a vague understanding of what a cold is and often confuse it with the influenza. As a common illness in daily life, people tend to overlook mild symptoms when they appear. There are two types of colds: the common cold and influenza. Because their symptoms are quite similar, misdiagnosis or missed diagnosis can easily occur.

The Difference Between Cold and Influenza

A cold, also known as the common cold or “shang feng” (a traditional Chinese term), differs from the flu, which is short for influenza. Although their names are similar, they have different pathogens and epidemiological characteristics.

Causes and Transmission

The common cold can be caused by a variety of pathogens, including multiple viruses, mycoplasma, and some bacteria. The primary viruses responsible are rhinoviruses, coronaviruses, and parainfluenza viruses. Since different pathogens can cause a cold, it can occur year-round. These viruses exist in the respiratory tract of infected individuals and are transmitted to others through droplets released when the patient coughs or sneezes. However, the common cold is far less contagious than the flu. Typically, a person is more susceptible to catching a cold when their immune system is weakened due to exposure to cold weather, getting wet in the rain, or overexertion. As a result, colds tend to occur sporadically rather than in large outbreaks like influenza.

Influenza, on the other hand, is an acute respiratory infectious disease caused by the influenza virus, which includes three main types: A, B, and C. These viruses also reside in the respiratory tract of infected individuals and spread through airborne droplets when the person coughs or sneezes. Flu outbreaks are most common in winter and early spring. One significant characteristic of the influenza virus is its ability to mutate frequently, leading to the emergence of new viral strains. Because of this, even people who have had the flu before can get reinfected during a future outbreak. Due to its high contagiousness, the flu can lead to widespread epidemics.

Symptoms and Progression

The incubation period for the common cold is about one day, and its onset is relatively gradual. Early symptoms include a dry and itchy nose and throat, mild fever, nasal congestion, runny nose with clear mucus, sore throat, dry cough, and hoarseness. Some individuals may also experience mild systemic symptoms such as chills, fatigue, headache, muscle aches, and loss of appetite. Fever is generally mild, around 38°C (100.4°F), but in cases of secondary bacterial infection, it may rise to around 39°C (102.2°F). After two to three days, nasal discharge thickens, coughing subsides, and symptoms gradually disappear. The common cold is usually mild and not life-threatening.

In contrast, the incubation period for influenza ranges from a few hours to two days, typically around one day. Flu symptoms often appear suddenly, starting with chills and shivering, followed by a rapid onset of fever, with body temperature rising above 39°C (102.2°F). Other symptoms include headache, muscle aches, extreme fatigue, dry throat, sore throat, coughing, phlegm production, and a runny nose. In general, flu patients experience more severe systemic symptoms than respiratory symptoms. High fever usually lasts for three to five days before gradually subsiding. Compared to the common cold, influenza is a much more serious respiratory illness that poses significant health risks, especially for the elderly and those with chronic respiratory conditions or heart disease. These individuals are at higher risk of developing pneumonia, which can lead to complications and even death.

Treatment and Prevention

For common cold patients with mild symptoms, medication may not be necessary, and rest is the primary treatment. If symptoms such as runny nose, sneezing, coughing, and phlegm production are severe, expectorants and cough suppressants may be used. In cases of persistent high fever and body aches, physical cooling methods or a combination of traditional Chinese medicine and Western fever-reducing pain relievers can be considered. Patients should rest adequately, drink plenty of fluids, and allow their bodies to recover naturally. Antibiotics are generally not required unless a secondary bacterial infection occurs.

For influenza patients, bed rest is essential, along with easily digestible foods and increased fluid intake. It is crucial to monitor for complications, such as pneumonia, heart failure, or convulsions. If any of these severe symptoms arise, the patient should seek immediate hospital treatment to avoid delays that could worsen the condition. Due to its high transmissibility, flu patients should be isolated and treated as soon as possible.

Prevention is key for both the common cold and influenza. Since influenza is a highly contagious respiratory disease, people should avoid gatherings or mass events in flu-affected areas. During flu outbreaks, wearing a mask when going out can reduce the risk of infection. General preventive measures include staying warm in cold weather, engaging in regular physical exercise, and boosting the immune system. During peak flu seasons in winter and early spring, it is advisable to avoid crowded public places whenever possible. One of the most effective ways to prevent influenza is to get vaccinated in advance.

A cold is one of the most common respiratory illnesses and can occur at any time.
