Patient: female, 20 years old
Date: 23rd May, 2023, first visit
Main Problem: Left rear heel pain for 1 week
I learned from patient that she needs to stand and walk a lot for a long time because of work. She also has not enough rest recently and then her rear heel starts to pain. She thought it will gradually be fine after a few days but it has gotten worse after 1 week and difficult to walk, so she came for treatment.
【Diagnosis】
During checkup, when the hand touches the affected area, there is a feeling of heat and slight swelling, by soft touches patient mentioned it is very painful accompanied by slight tingling sensation.
When palpated upwards from the affected area, the skin of calf was not hot and not painful, but the calf muscles were tight.
After basic diagnosis, strain on rear heel that is damaged and is currently in an inflammatory phase are the causes of pain.
After a preliminary examination and diagnosis, patient was asked to the treatment room and lay down on the treatment bed for treatment.
【Treatment】
Try to avoid tuina and pressing too hard on painful part during treatment as the part is still in inflamed stage. It will aggravate the pain if tuina or pressing too much on the inflamed part.
After both tuina and acupuncture together, the patient is asked to get up and try to walk around.
“The patient felt that the pain while walking was slightly relieved, but there was still pain.”
“This is a normal phenomenon because it is currently in the stage of injury and inflammation, your heels need to support the body weight when walking so there will still be slight pain, but the condition is much better than before treatment and it needs to take 2-3 days to recover.”
“I will prescribe some traditional Chinese medicine for you. This medicine can help you dredge the meridians, activate blood circulation, and regulate your body. In addition, it needs to be combined with external patches to strengthen and repair of the injured part.”
Before the patient left, I told her again: “Try to rest as much as possible in the next few days, walk slowly and don’t stand for too long, remember to take the medicine and apply it on time, the pain will be relieved afterwards.”
Follow-up visit: May 27, 2023
After 4 days consuming Chinese medicine and external patches application, the patient mentioned there is no pain around rear heel area and patient can walk properly when the patient came back for follow-up visit.
The patient was asked to lay on treatment bed for an examination and found that the condition of rear heel was much better. There was no inflammation and no pain around the affected area when pressing.
This time, tuina and acupuncture were used to consolidate the curative effect, helping to warm the meridians and relax the legs muscles.
After the treatment, the patient felt relieved when walking.
【Rear Heel Injury Treat Tips】
1. Touch the injured part and feel the local skin temperature. If the part feels hot and more painful when touched, you can use ice water bag for 10-15 minutes to relieve inflammation and pain.
2. Conversely, when we touch the injured part and feel that the local skin temperature is normal and the local muscles feel tight or sore, we can use a hot water bag for 10-15 minutes, we recommend not exceeding 45°C to avoid scalding. Hot compresses can relax tense muscles and promote local blood circulation.
3. After the rear heel is injured, rest as much as possible and move less, so the injured part is not burdened and aggravate the condition.
